OHCOBR will begin using the data reporting tool on October 1, 2022.
I don’t see the reporting tool on the website, where do I find it?
For security reasons, the reporting tool is not published on the website. For access, please complete the access request form or send an email to [email protected].
My company’s point of contact for grievances and appeals reporting has changed, how do I inform you of this change?
Please send an email to [email protected] and provide the name and contact information for the new POC.
Will you still accept the old reporting form?
No, reports will only be accepted through submission of the new data reporting tool.
